You do not need a sick note from a doctor if you are off work for less than 7 days. In these situations your employer may ask you to complete a self-certification form. This form is available from your employer or on-line at HMRC.
However, for any length of time beyond 7 days a sickness certificate will be needed. It is important that you see one of the doctors as soon as possible, as we are unable to issue back dated certificates.
If you have been discharged from hospital and have a discharge letter confirming your illness ( for example, a broken leg or a heart attack) please telephone to speak to a doctor. This should avoid the need for you to make an appointment.
In the rare situation where an employer insists that you have "proof of illness" for periods of less than 7 days, we will issue a "private " sickness certificate at a cost of £12.00. This does however use up GP appointments un-necessarily and we would urge you to get your employer to accept the self- certificate.
Fit for Work Service

The Fit for Work Service is a free service for individuals who are off work sick, and who are struggling to go back to work and deal with their health issues. We can provide access to a range of health and other services tailored to help you return to work as quickly as possible. To be eligible for the service, you must be employed, and either be living or working in the Royal Borough of Kensington and Chelsea, London Borough of Hammersmith and Fulham, or the City of Westminster.
