If you’ve been off sick less than seven days

The NHS provides sickness certification only after seven continuous days of illness. If you need a sick note to cover a period shorter than this, you can complete a Self Certification (SC2) form, available here or from your employer.

If you’ve been off sick more than seven days

Your employer may ask for an NHS sickness certificate. It is best to contact the surgery using PATCHs for this, we will get back to you as soon as we can.

Do I need a note saying I’m fit for work?

No. You do not need to see your GP again to be ‘signed back to work’.
